Your initial appointment will consist of a consultation and complete evaluation explaining your diagnosis and the treatment options. This initial consultation affords you an opportunity to discuss all aspects of your care with the doctor and make all necessary arrangements in advance of your surgery. On occasion, surgery may be performed on a same day basis but a complex medical history or treatment plan will require a second appointment for the procedure to be completed.
Please assist us by providing the following information at the time of your consultation:
• Your dentist’s or physician’s referral.
• Bring any X-rays available.
If your dentist or physician has taken or ordered x-rays or other imaging studies you should request that they be forwarded to our office. Please do this well in advance of your scheduled appointment. If there is not enough time, please pick them up and bring them with you to our office. Failure to have the needed imaging available will result in delay, rescheduling and additional cost, as new x-rays will need to be taken.
• A current list of all medications you are presently taking.
• If you have medical or dental insurance, please bring the necessary information. This will save time and allow us to help you process your claim efficiently.
IMPORTANT: All patients under age 18 must be accompanied by a parent or legal guardian for all appointments.
